We then facilitate a face to face intervention to assist your loved one in committing to treatment at the proper level of care with a respected treatment provider.

Care Management is crucial in the process of treatment. After a program is complete it is vital to have aftercare services in place prior to discharge. Our Care Management teams will stay in weekly contact with the individual who has just graduated from an inpatient level of care. We not only stay in contact with the individual, but also with their aftercare facility and family members. We offer this on a month to month basis or for the year depending on what the individual requests.
