What happens after treatment at the inpatient level of care? A New Way of Life Intervention Services offers care management on a month to month basis or 3 months, 6 months and a year at a time. We offer this service to support those leaving the inpatient level of care to return home or to sober living. Long term success lies in having as much added support as possible, ongoing. Our Care Management team is here for you and your loved ones to help follow their progress ongoing. We want to know what is working for them and what is not working for them. Consistency is important to us as well as our clients, because it builds trust and opens honest dialog. We are here to support you, and help you keep on a clear path moving forward.

We will keep in touch with key organizations, family members and the person struggling, for as long as you would like us to. Care Management includes weekly calls with our clients, face to face meetings as necessary, 24/7 access for emergencies and communication to such places as courts/probation/IOP/Sober living facilities. Please do not hesitate to ask us specific questions related to your individualized journey. It has been proven that this advocacy with trusted partners will ultimately help you and your loved one maintaining your New Way of Life.
